Beginning supervised practice before the Board approves the supervision agreement — hours logged before approval may be invalidated
Missing the 36-month provisional license expiration — if you haven't completed requirements, you must apply for a 24-month renewal before expiration
Not submitting quarterly supervision logs on time — the Board requires regular documentation of hours and supervisor assessments
Assuming calendar time alone satisfies the requirement — you must complete both 3,000 hours AND 24 months independently
Using a supervisor who lacks ALPS designation — only Approved Licensed Professional Supervisors with 2+ years licensure and 5+ years experience qualify
